In her video, Sanchez dances in a hospital hallway while tethered to her IV pump, and she said she loves Drake's music and asks if he can come cheer her up for her birthday.
In a video made by the hospital, Sofia screams "OH MY GOD" upon seeing Drake.
Drake visited a young fan named Sofia Sanchez in a Chicago hospital on Monday.
Drake, in a surprise visit, granted one of two of Sanchez's birthday wishes (the other being to receive a new heart).
Sofia has been in the Lurie Children's Hospital in Chicago for several weeks as she was born with cardiomyopathy and needs a heart transplant.
Drake was in town to perform at the United Center as part of his "Aubrey and the Three Migos" tour at the United Center on Saturday and Sunday night.
Earlier this year Drake surprised students in Miami while he was filming the video for single "God's Plan", donating $25,000 (£19,480) to the Miami Senior High School where he shot some of the video. She now requires a heart transplant to survive.
Not only did Sanchez get a visit from Drake (something girls across the world dream of), she was also seen repping some Scorpion merch in another photo posted to his Instagram. "Me and my love Sofia talking about Bieber and Owls and Basketball", he captioned a photo smiling wide next to Sofia. The resulting heartfelt moment was captured by the staff at Ann & Robert H. Lurie Children's Hospital, who shared the footage on Instagram.
"Having this one special day where she doesn't have to be a sick patient, it's just a little girl who's a fan of a great artist, is pretty special", said Lurie Hospital Pediatric Cardiologist Dr. Bradley Marino.
